I am a cash-based practice. I do not accept any insurance, including Medicare or Medicaid. I am categorized as “out-of-network” providers for insurance plans. If requested, I will provide the documentation required for you to bill your insurance company, and you will be reimbursed according to your out-of-network benefits. If you are unsure what your benefits are, please check with your insurance company prior to your appointment. In addition to my professional fees, there are the lab tests. Even though we are “out of network”, the labs I order maybe in your network.
